	Application to use Holgates Green	If you wish to use Holgates Green to hold an event please complete the following form. We will contact you further when consideration has been given to your request.

	Change of Address Notification	Please complete and submit this form if you are changing your address. The form should be completed by the person leaving the address. The information you give will enable the Council to change your records for Council tax, water rates, non-domestic rates, water metering, social services, and any other contacts you have with the Council.

	Compliments, Comments or Complaints for the Council	Use this form to send any compliment, comment, or complaint about the services and facilities provided by the Council of the Isles of Scilly.

	Contact the Council	We welcome your comments, queries and any questions you may have on any aspect of the services of the Council of the Isles of Scilly. 	Freedom of Information Request Form	Please complete the following form providing as much detail as possible about your request, to enable the Council to identify and locate the information you seek. The scheme and this legislation does not limit in any way other rights that people have to information nor does it limit in any way the very large amount of information that we otherwise publish outside the scheme. Therefore planning applications will continue to be available for public inspection. Again, the rights of our tenants and housing applicants with regard to information concerning their tenancy and application are also unaffected.

	Library Book Reservation	If you would like to reserve a library book from the Cornwall online service, please complete this form.

	Lighthouse Email Sign-up	If you would like to receive an electronic copy of the Lighthouse just enter your email address below. We will send you each edition as they become available.

	Missed Refuse Collection Report	Please fill in the form below if your refuse has not been collected on the normal day of collection. If the Council receives the form by 5pm, the missed refuse will be collected the next day, if this falls on a weekday.

	Planning application comments form	You can use this form to send your comments on any planning application. Alternatively you may send comments by letter or email using the contact details on the information page about planning applications

	Reporting Of Street Light Failure	Use this form to report the failure of street lights.

	Reporting Graffiti	Please complete this form to report any graffiti you have seen on the islands so that it can be removed.

	Reporting Parts Of The Highway In Need Of Repair	Please use this form to report roads and pavements which are in need of repair.

	Reporting Vandalism	Please fill in this form to report incidences of vandalism

	Rodent Controller Visit	Please complete this form if you need a visit from the rodent controller to deal with problems you are having with rats or mice. Your name will be added to the list and you will receive a visit when the Rodent Controllers are next on the islands.

	Taking over the tenancy	Taking over the tenancy of a Council home from someone who is still alive: you can apply to take over the tenancy from someone who is moving out of the home - it could be an elderly relative who has moved to sheltered accommodation for example. The Council does not legally have to agree to your request - it depends on your circumstances and whether the property could be more suitable for another type of household (like a family for instance). But we will not turn it down without a good reason. Please fill in the form below.
